Overview of Positions

The Presbyterian Medical Services Care Coordination Program supports and empowers clients and the healthcare team(s) caring for them to improve client outcomes and experiences within the healthcare system.

The PMS Care Coordination Program assists New Mexico Medicaid clients with care navigation, removing barriers to care and takes a proactive approach to health promotion, education, and disease management. In addition, as needed, they connect their clients with shelter, food, transportation, and other key resources to enable healthy lifestyles. Our goal is to help improve the quality of care while reducing the total cost of healthcare for clients.

Most PMS Care Coordinators work in a hybrid-remote setting either in PMS Health Center or at the PMS Engagement Center in Albuquerque. In some instances, Care Coordinators may also be responsible for traveling to a client’s home to provide services.

Some positions include: Community Health Worker, Care Coordinator I, Care Coordinator II, Care Coordinator II – Licensed, Care Coordinator III, Care Coordination Assistant Manager, Director of Care Management
